No. Due to COVID restrictions, as of March 15, 2021, we are still not seeing clients in person.

Although we are not open for in-person appointments at this time due to COVID restrictions, you can call (315) 458-7077 during office hours: Monday through Thursday 9 am – 4:45 pm and Friday 8:45 am – 11:45 am.

We can mail an application to you, fax it, or send it by email.


You can leave your paperwork in the drop box outside the office door at 110 Singleton Ave., North Syracuse, NY 13212. It is secure — only our administrator and staff have a key. The USPS does not have access to this box.

As of March 2021, we are still quite behind. You should receive your paperwork within 120 days prior to your actual recertification date.

When Will I Get Paperwork For My Recertification?


We will send out an employment verification form to your employer. (If your employer has changed, let us know your new information.) You will need to sign an Authorization to Release Information form. We can mail, fax, or email the authorization form to you.

My Income Changed, What Do You Need Me To Do?


For persons added to your household, we will need birth certificates, social security cards, and photo ids (where applicable). If someone moves away, please notify us. If someone passes away, we will need their death certificate.

It could have increased due to a few factors: income changes, allowance changes (for seniors), Fair Market Rent amounts changed, rent increases, etc.
